Process for submitting applications: Interested students 12-17 years of age must complete their portion of the application as well as have their parent or guardian complete and sign in the appropriate places. Youth 18-19 years of age can complete the application themselves without a parent or guardian's signature.
Session start dates are announced in advance. Applications should be submitted prior to the beginning of the fall and summer session of a respective year. After completion, applications can be submitted by fax at 504-376-2370, email at email@example.com, delivered in person to the band officer or by mail to: 1233 Westbank Expressway, Band Room, Bldg E, Harvey, LA.
The application (Social security number must be on the form) AND signatures in all places or the paperwork is considered incomplete and must be held until it is complete. This means that your child can not become a part of the band until they are processed.
